Automatic capture management (ACM) algorithms are extremely reliable and routinely used in cardiac resynchronization therapy devices including left ventricular (LV) leads. We report a case of failure ACM patient with biventricular (BiV) ICD unusual late dislodgement bundle branch pacing (LBBP) lead. The present highlights unique challenge patients BiV LBBP lead the port who programmed to subthreshold right (RV) outputs promote only RV fusion. A 74-year-old man nonischemic cardiomyopathy, LVEF 20% LBBB underwent successful BIV utilizing LV (Nov 2020). output was pacing. threshold stable at 1V 6-month follow-up which time had normalized. atrial During remote 1 year (Dec 2021) it noted that device stopped assessing threshold/impedance on August 2021(blue circles). Presenting rhythm demonstrated spikes without but conducted beats from sinus (AS) sensed as VF event (arrow). not attempted by due ongoing AS-BP-TF resulting tachycardia detection count 20/40.
